Transition of Care

Transition of care is essential to members with complex care needs. Our case managers handle the coordination and continuity of healthcare to ensure patients receive the most efficient, effective care possible. Currently, this program focuses on patients with heart failure or who have had heart bypass surgery.

Transition of Care Services include:

• Patient and family education
• Health professional coordination
• Care follow-up
• Drug interaction review
• Additional services deemed necessary by case managers

Transition of care is available to Blue Cross and Blue Shield of Alabama members at no additional charge.
