Here’s How We Can Help

The Serious Illness Care Program offers extra care and support focused on you. You will have a nurse who calls you to:

  • Help with pain and other problems
  • Schedule (coordinate) your doctor appointments
  • Talk about what matters to you
  • Teach you about your condition
  • Support you and your caregiver

If you become seriously ill, only you will know what matters most to you. It’s important to share these thoughts with your doctors and loved ones so they can give you the care you deserve, even if you can’t speak for yourself.

Here are three ways to share what you care about:

  1. Choose a Trusted Partner: Pick someone who can speak for you if you can’t
  2. Fill Out a Living Will: Write down your wishes in a living will. If things change, you can update it
  3. Talk to Your Doctor: Tell your doctor what matters most to you and who will speak for you if needed
